LIABILITY RELEASE AND ASSUMPTION OF RISK
I/We understand that participation in workshops organised by Flawless Dance Hub could result in potential personal injury. Despite precautions taken by Flawless Dance Hub, accidents and injuries may occur. By agreeing to and signing this release form, I/We (the dancer/participant/parent/guardian) assume all risks associated with the use of any and all spaces/venues/locations used by Flawless Dance Hub.
I/We agree to release Flawless Dance Hub, including all choreographers, dancers, staff members, and facilities used by the organiser, from any responsibility or liability for any cause of action, claims, or demands now and in the future. I/We will not hold Flawless Dance Hub liable for any personal injuries, including but not limited to scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, or death, nor for any personal property damage or loss that may occur on the premises before, during, or after the workshops.
Furthermore, I/We agree to adhere to the workshop and facility rules and accept full responsibility for my/our behaviour, as well as any damage I/We may cause to the facilities utilised by Flawless Dance Hub. I/We understand that Flawless Dance Hub reserves the right to remove any participant from workshops or events without refund if they are disruptive, rude, abusive, or fail to comply with instructions from staff, event organisers, or contracted personnel.
PHOTOGRAPHY/VIDEO
- I/We understand that recording of workshops/auditions is strictly prohibited.
- I grant permission for photographs/videos of myself/my son/daughter to be used for publicity and advertising purposes by Flawless Dance Hub, including in their studio, on their website, and on social media channels.
ACCEPTANCE
By submitting this registration form, I confirm that I fully understand the terms of this waiver and have agreed to it voluntarily. I affirm that I am over 18 years old, or if under 18, this waiver has been signed by my legal guardian. I agree that this waiver is binding on my heirs, next of kin, executors, administrators, and successors.
